Format a Survey
Click survey formatting from the upper right of your survey's main page.
Introduction to the Survey Formatting Page
- Email Response: Automatically send an email to a user after they have completed your survey.
- Post Survey Page: Customize the page your survey users will see once they complete the survey. This is a good opportunity to allow a user to review the survey questions and their answers.
- Credit Card Payment (not shown): County sites can create surveys that accept credit card payments for event registration. If you operate a county site and would like to use this feature, please contact Karl Krist.
- Move/Copy/Disable/Delete: Move your survey to another site, copy it to a clean survey with no data, disable it or delete it completely.
- Survey Name: Change your survey's name.
- Notification Email Address: If you would like an email to be sent out each time someone completes the survey, enter the recipient's email address here.
- Add to Portal: Enter the email of anyone in the ANR directory who might need administrative access to the survey.
- Survey Spam Block: Enabling this feature will require your users to enter a simple, random code prior to completing the survey.
- Text at Top of Survey: Use this text box to introduce your survey, provide instructions, list dates and locations, etc.
- Upload a File: Allows users to attach a file to the survey.
- Automatically Number Questions: Numbers your questions in order.
- Quiz Save Button Text: Allows you to create a custom button for Quiz only.
- Divider Between Questions: Creates a rule between questions.
- Custom Header: Allows you to upload a custom header graphic.
- Quiz & Score Quiz: You can change your survey to a quiz at any time. When the survey becomes a quiz, you will need to revisit any questions you created to ensure that they have correct answers marked.
- Submit Button Text: You can customize the submit button if you would like it to reflect the type of survey you have created.
- Password Protect Survey: Set a password here and your users must enter it prior to taking the survey.
- Encrypt Results: Check the box and set a password here, and the data stored in our database will be encrypted.
