Do you want to put a survey on your web site? Do you want to collect information or gather feedback from your clientele online? Maybe you’d like your web site to include a quiz to test the knowledge of your users. All these options are easy now for counties, workgroups, or programs using the Site Builder tool.
Site Builder now contains a Survey Maker that allows you to create questions with yes/no answers, drop-down lists, check boxes, multiple choice, and short or long answer text. Then you simply create a link to the new survey on your web site. Surveys can be made "secure" with the click of a button, allowing you to request data from your users with confidence. At any point you can go back into Site Builder to view the responses received. You can also create an Excel file with the responses that you download to your computer, and set up the Survey Maker to allow people to upload files to you.
Since its completion last fall, the Survey Maker has been used to create conference registration forms, requests for proposals, course quizzes, and event evaluations. Here is an example of a quiz that accompanies an online course from UCCE Placer-Nevada County: http://ucce.ucdavis.edu/survey/survey.cfm?surveynumber=43&back=none
