New to 4-H Adults
 
2008-2009 Adult Volunteer Application Forms

The first step is to view the orientation DVD and take and pass the quiz.

After completing the county orientation and quiz, you will be contacted by the   4-H Office regarding locations where you may have your fingerprints taken. 

After the 4-H Office receives the results of your live scan prints and background check by California Department of Justice, we will contact you. 

When you are notified of your DOJ clearance, please print this packet 2008-09 Adult Volunteer Enrollment Application , complete, and bring to your club registrar at your next club meeting.

Call your club leader to find out if they have club fees in addition to the CountyProgram fees and State 4-H Accident/Sickness Insurance and Program Fees.

Make all enrollment checks payable to your club.
 
Returning Pre-approved Adults

Retuning Adults Not on Club Roster
 
It has come to our attention you have not completed the required 4-H orientation, fingerprinting and background clearance. There are new procedures in place for the 2008-2009 year that must be completed prior to joining a club.
